Pursuant to Federal Law No. 152-FZ of 27 July 2006 “On Personal Data”, I hereby confirm that I am a personal data subject and, where applicable, the lawful representative of other personal data subjects from whom I have obtained consent to the processing of their personal data and its transfer to third parties. Acting on my own behalf, of my own free will and in my own interests (as well as on behalf of, in the interests of, and with the will of other personal data subjects whom I am officially authorised to represent), I knowingly give my consent to LLC “Sai-Khonash Travel” (INN 1701057310), registered office: Russian Federation, Republic of Tuva, Kyzyl (hereinafter – the Company), website: tuva-travel.com, to process the following personal data:
surname, first name, patronymic;
place of birth;
telephone numbers;
e-mail addresses;
postal addresses of personal data subjects;
number of the primary identity document, date of issue and issuing authority;
passport data for international travel, and, where necessary, copies thereof;
any other personal data obtained from me (including of the personal data subjects whom I officially represent);
user data (location information; type and version of the operating system; type and version of the browser; device type and screen resolution; referral source and advertising reference; OS and browser language; pages visited and buttons clicked; IP address);
any data concerning me that become known in the course of contract performance (if a contract is concluded between me and the Company).
By providing these details I warrant that they are accurate, that I possess them on a lawful basis, and I undertake to provide documentary proof thereof at the operator’s request.
The website collects and processes anonymised data on visitors (including cookies) using web-analytics services (Yandex.Metrica, Google Analytics and others).
Purpose of processing: handling incoming enquiries from individuals for the provision of travel services; receiving requests and applications; consulting on issues relating to the Company’s business activities; conclusion and performance of contracts; analytics of user actions on the website and ensuring website functionality.
I consent to any action or set of actions with all the specified personal data in accordance with clause 3 of article 3 of Federal Law No. 152-FZ “On Personal Data”, whether performed with or without automation tools, including collection, recording, systematisation, accumulation, storage, rectification (updating, modification), retrieval, use, transfer (distribution, provision, access), depersonalisation, blocking, deletion, destruction of personal data. The data are processed for the purpose of obtaining information about the services provided by LLC “Sai-Khonash Travel”, concluding service contracts for the sale of travel products and additional services supplied by the Company, and to exercise the rights and legitimate interests of the personal data subjects, the Company or third parties, in compliance with the conditions for processing personal data set out in article 6 of Federal Law No. 152-FZ.
I hereby acknowledge and confirm that the Company is entitled to entrust the processing of personal data of citizens to third parties under agreements concluded with such parties when necessary for booking travel products and fulfilling the User’s order, namely: airlines; tour operators; hotels; visa centres; consulates; courier services; postal operators; internet service providers; organisations providing e-mail distribution services. Persons processing personal data on behalf of the Company undertake to observe the principles and rules of processing and protection of personal data established by Federal Law No. 152-FZ of 27 July 2006. For each such person, the list of operations with personal data, the purposes of processing, the obligation to maintain confidentiality and ensure security of personal data, as well as the protection requirements, are defined.
This consent is given by me personally and remains valid until it is revoked by sending a written notice, personally signed by me, to LLC “Sai-Khonash Travel” at 667003, Russian Federation, Republic of Tuva, Kyzyl, 13 Gornaya St., apt. 84, by registered mail with delivery confirmation, or by hand against signature to a duly authorised representative of the Company. The date of revocation shall be the day following the day on which the Company receives the written notice of withdrawal of consent.
Upon receipt of my written notice withdrawing this consent, the Company must cease processing my personal data in terms of collection, recording, systematisation, accumulation, rectification (updating, modification), retrieval, use, transfer (distribution, access).
By providing this consent I confirm that I and the personal data subjects named by me on the website tuva-travel.com and in the contracts concluded with the Company have read and understood the Personal Data Processing and Protection Policy of LLC “Sai-Khonash Travel” and Federal Law No. 152-FZ “On Personal Data”, and accept the terms of the User Agreement.
If the data specified in this consent are inaccurate or if I lack lawful authority to provide data of other personal data subjects, I undertake to cease using the website tuva-travel.com and refrain from concluding contracts on behalf of third parties.
I have been informed that I have the right to access my personal data, to request the rectification (updating, modification) or deletion and destruction of my personal data where their processing by the Company violates my lawful rights and interests or the legislation of the Russian Federation.
I acknowledge that if I withdraw this consent, the Company may continue processing my personal data without my consent where grounds specified in sub-clauses 2–11 clause 1 article 6, clause 2 article 10 and clause 2 article 11 of Federal Law No. 152-FZ exist.
I undertake, in the event of any change in my personal data, to promptly inform the Company and provide supporting documents.
By this Consent I confirm that I am the subject of the personal data provided and that the data supplied are accurate.
